Has anyone figured out where to go to request a MS SQL DB to be created?

You can create your db in your control panel in real time. You don't need to request it.

I agree. I still haven't isn't obvious to me either how to create a MsSql Database. Could you please give step by step instruction.

I'll answer my own question:

control panel >> Database >> Database Admin (Right hand side) >> MSSQL Admin >> create new DB (AT TOP OF PAGE)
